Memorial Home Health has again been recognized for Excellent Care.
Memorial Home Health has been named to the Top 500 of the 2016 HomeCare Elite®, a recognition of the top-performing home health agencies in the United States. This is the fifth time that Memorial Home Health, a service of Memorial Hospital, has received this recognition. It is the only Union County home health agency to receive this distinction this year.
HomeCare Elite® identifies the top 25-percent of agencies, which are ranked by an analysis of publicly available performance measures in quality outcomes, best practice implementation, patient experience (Home Health CAHPS®), quality improvement and consistency, and financial performance.

The team of skilled home health nurses, certified home health aides, and therapists monitor a patient’s health, assist in the recovery process, or provide care for those who cannot easily travel for medical care. Patients age 14 or older may benefit, including those needing maternal health services. Assistance with housecleaning, errands, personal care, and grocery shopping is available for those who qualify.
