Memorial Sports Medicine is proud to offer free pre-participation physical exams for your athlete – now available year-round! All local athletes (incoming 7th–12th grade students) are able to receive their physical with our Memorial team.
Checklist to schedule a free sports physical from January to December: (Choose option 1 or 2):

You can schedule a free sports physical with your child’s provider by following these steps:
- Your child must be an established patient of the Memorial practice.
- Call your provider’s office directly to schedule. Go here to see a list of our practices. Appointments are required.
Please note:
- If your athlete has had their well check visit/physical in the last 12 months, your provider may be able to complete the sports physical paperwork for you without an appointment. Call the office to discuss next steps, as it can take 24-72 hours to complete.
FOR BOTH OPTIONS:
Parents/guardians must complete the Ohio High School Athletic Association (OHSAA) Pre-Participation Physical Exam Form prior to the physical appointment. Forms can be obtained from your school athletic department or by visiting here.
Parents are encouraged to attend the physical with their student athlete. Physicals can only be performed if paperwork has been completed/signed by a parent or a parent is present.

1. Does Memorial only offer sports physicals in the June-July time?
Memorial now offers free sports physicals year-round for student athletes in our region. Appointments are available January to December!
2. Why does my student-athlete need a sports physical?
The Ohio High School Athletic Association (OHSAA) requires all student-athletes to complete a Preparticipation Physical Evaluation (PPE) for the school year. This evaluation ensures that all athletes are physically prepared to safely participate in sports activities. The PPE form must be completed by a licensed healthcare provider and submitted to the school before students can engage in any athletic practices or competitions. Parents and guardians are encouraged to schedule these evaluations promptly to avoid delays in their child’s participation in school sports.
3. When are sports physicals due?
Physicals are due on the first official day of the athlete’s sport, and must be completed/returned to the athlete’s school once every 13 months (12 months if completed in May/June) before any athletic participation. Typical deadlines:
- August 1 for all fall sports
- October 25 for girls basketball, hockey, swimming and November 1 for boys basketball, bowling, gymnastics
- February 17 for softball and February 24 for baseball, lacrosse, tennis, and track
It is suggested to schedule sports physicals at least 6 weeks before the sports season starts. If your doctor finds a problem, this should leave enough time to for tests or treatment, if needed.
4. Does my child need a well-check visit too?
A sports physical is not intended to replace regular or as needed visits with your student athlete’s primary care physician. The sports physical focuses on well-being as it relates to playing a sport. Regular checkups are still important for your child’s overall health and well-being. If you plan ahead, the doctor may be able to do both types of exams during one visit. When scheduling your annual visit, mention you need a sports physical and a regular checkup so your doctor can set aside time to do both.
