Our Program:

The Memorial Hospital Pharmacy Residency is a new program with an anticipated start date of July 2025. The residency has an ambulatory care focus with the majority of the program being based in the Medication Therapies Center (MTC). The Medication Therapies Center is located in our newly-constructed Outpatient Pavilion, attached to the inpatient hospital.

Pharmacists at the MTC work to provide Disease State Management, Medication Management, and Transitions of Care services to patients of Union and surrounding counties through collaborative practice agreements with primary care and specialist providers of the region.

Specific clinic offerings include anticoagulation, COPD/asthma, diabetes, heart failure, lipids, weight management, medication therapy management (comprehensive medication reviews), and transitions of care. While the core of our residency program will occur in the MTC, a staffing component in the inpatient hospital will provide residents with some acute care and inpatient training as well. Rotation electives will be available in both settings.

Setting Your Expectations

Memorial will be pursuing full accreditation during the first year of the residency program. Once accreditation is gained, it becomes retroactive to the date the application was filed with ASHP, so the current resident would be provided with a certificate indicating completion of an ASHP- accredited residency program.
