Diabetes Education

Our registered dietitians and staff provide counseling and individualized nutrition plans which are healthy for diabetics. You will learn what you should and should not eat to improve your health and help control your diabetes.

Appointments can be made by your physician's office or yourself. You may want to verify with your insurance carrier that medical nutrition therapy is or is not a covered service under your plan. If you would like to have your insurance carrier billed for the medical nutrition therapy consult, a physician order, including diagnostics, must be received by the time of your appointment. Your physician's office can fax a referral directly to our office at (937) 578-2371.

If you would like to self-pay for your appointment, a physician order is not necessary.

Please plan to arrive 15 minutes early for your scheduled appointment In the Food and Nutrition office, to complete necessary paperwork.

Free Pre-Diabetes Education Classes are held regularly at Memorial Hospital, where our registered dietitians provide information about pre-diabetic symptoms, how to tell if you may have diabetes, and the role of nutrition and exercise.
