What’s more is, you may already use MyChart with other providers, and now you can connect with your Memorial providers – including Memorial Hospital and Memorial Medical Group physician offices.

Make appointments at your convenience, complete pre-visit tasks from home, and find the nearest urgent care or emergency room when you need it.

NEW FEATURE: Waitlist! Patients new to a practice or clinic may receive a MyChart® notification alert when an earlier appointment becomes available. Patients who accept these offers are then able to schedule with their provider sooner!

Send a message, check health summary information and test results, or arrange to follow up with a doctor, depending on the level of care you need. Plus, you can submit your prescription refill requests!

Access your health records from other healthcare organizations, too! You can link your Memorial MyChart® account with other MyChart® accounts. Plus, you can use the same log-in information for your OSU MyChart®.

For medical records requests, please contact our Medical Records Department at Memorial Hospital – our team can be reached at (937) 578-2365 from 8:00am to 4:30pm, Monday through Friday. Alternatively, medical records requests can be submitted through MyChart.
