Registration at Memorial Health locations.
We make every effort to streamline your registration process at Memorial Hospital and all locations within Memorial Health. Wherever you arrive to register, please bring your:
- Insurance card
- Photo ID
- Medical Power of Attorney papers, if applicable
- Advance Directive papers, if applicable
For Outpatient Surgery at Memorial Hospital (Entrance F) or Memorial Hospital Outpatient Surgery Center (122 Professional Pkwy, Marysville)
Your surgeon’s office will inform you of the location for your procedure, and the registration process. You will be advised if you need to visit the hospital prior to your procedure for pre-admission testing.
For registration, one of our highly trained professionals will ask you a few questions and help you complete the necessary paperwork in strictest confidence. Once your registration is complete, you will be given an identification bracelet. If you have questions, call (937) 644-6115 or (800) 686-4677, and ask for Registration.
If you or your family members have any questions before your surgery call, (937) 578-2476.
For Childbirth
- If arriving between 6am and 9pm, come to the Miracle Life Center (Entrance B) to register.
- If arriving between 9pm and 6am, come to the Memorial Hospital Emergency Department (ER Entrance) to register.
Following registration, you will be taken to the Memorial Miracle Life Center.
For all persons under the age of 18 who are receiving medical treatment:
For all persons under the age of 18, a Medical Consent Form (signed by parents or a legal guardian) is required, giving another party consent to make emergency medical decisions in their absence.
